Partial Hospitalization (PHP) Services
Transitions, Inc. offers Partial Hospitalization Services, which functions as a step-down program from residential services and may be used to prevent or minimize the need for a more intensive and restrictive level of treatment. PHP provides group services four days per week, five hours per day. PHP clients meet individually with their primary clinicians on a weekly basis. Clients in PHP are also expected to meet weekly with their Targeted Case Manager, who assists in breaking down barriers related to housing, employment, legal issues, and access to healthcare during the recovery process.

At Transitions, we believe that providing individualized treatment for Substance Use Disorders is the best path for success. Using evidence based practices, our expert staff will create a personalized treatment plan based on the clients needs. Residential and Outpatient treatment program length will vary from client to client.
