Position: Director of Recovery Housing

Status: Full Time

The Director of Housing effectively manages the agency’s rental property.. The Director of Housing is directly responsible for maintaining daily, weekly, and monthly reports including accurate reporting of rents and deposits received.

Duties and Responsibilities:

  • Conduct all business in accordance with company policies and procedures, Fair Housing, Americans with Disabilities Act, Fair Credit Reporting Act, and all other laws pertaining to apartments; Report regularly to the COO on the status of all properties, rent collection activity, leases and occupancy; Maintain maximum occupancy levels by working with internal programs and community agencies to minimize vacancies and disruption of occupants; and Ensure proper operation of housing units based on local ordinances and programmatic guidelines.  Income Collection:  Maintains accurate resident records. Updates on a daily basis all rents, deposits and application fees received from residents. Issue appropriate notices when necessary (i.e. late payments, eviction notices, returned check memos). Deposits all receipts prior to bank close each day. Resident Relations: Maintains positive customer service attitude. Physically inspects property when on grounds, picks up litter and reports any service needs to maintenance staff. Will also inspect move-outs and vacancies when requested. Marketing: Must be knowledgeable of all phases of leasing and resident retention. Works with lease renewals each month. Greets prospective clients, shows community and performs leasing duties as needed. Answers and handles incoming telephone calls from prospective new residents, current residents, vendor/suppliers, etc. Maintains awareness of local market conditions and trends. Contributes ideas to the Leadership Team for marketing community and improving resident satisfaction.  Administrative: Updates required reports concerning move-out notices, activity, etc., on a daily basis and provide information to the Property Manager.Organizes and files all applicable reports, leases and paperwork. Proofreads all lease paperwork. Processes all security deposit move-out reports. Accepts service requests from residents and routes to maintenance for prompt processing. Conducts service follow-up with resident when work is completed. Qualifications: High school diploma or GED; Previous housing management and supervision experience strongly recommended.

Position: Director of Quality Improvement 

Status: Full Time

The Director of Quality Improvement is responsible for developing, managing, and overseeing the agency’s quality and clinical standards and programs in accordance with government regulations, client specifications, certifying bodies, and internal requirements. This role has overall responsibilities for the development and implementation of the quality and performance improvement program. The incumbent in this position must have strong communication skills, and have an unwavering attitude to quality practices.

Qualifications-  Bachelor’s Degree in Behavioral Health Sciences. Position: Assessment Specialist

Status: Full Time

Position Summary:

The Assessment Specialist completes initial clinical assessment and advocates for patients from crisis intervention through admission, collaborating with the Utilization Review Care Manager and Clinical Director.  Job Responsibilities:  Complete clinical assessment for new intakes at the Residential Treatment Center and determine appropriate level of care recommendations; Assist new patients at time of admission through authorization process; Complete authorization for new admissions when needed including assisting in the peer to peer and appeals process; Fulfills productivity and documentation requirement on a weekly basis; Provides clinical coverage when needed, including but not limited to crisis intervention, group facilitation, treatment planning and individual counseling; Participates in quality assurance related to the intake and assessment process; Demonstrates an independent work initiative, sound judgment, diplomacy, tact and professional demeanor; Excellent verbal, written and/or interpersonal communication skills. Qualifications: Master’s Degree in Social Work, Clinical Counseling, or related field; Active Kentucky License as a Certified Social Worker (CSW) or Licensed Professional Clinical Associate (LPCA), Independent license preferred.

Position: Registered Nurse 

Status: Full Time
Provides nursing services at our multiple treatment locations. Works under the direction of the Clinical Treatment Director.  Performs comprehensive assessments during admissions process to assist in the plan of care, identifying appropriate pathways based on client history.  Provides nursing education based on client’s individual needs to promote well-being.  Observes medication administration, monitors for adverse drug reactions or severe withdrawal symptoms and completes documentation in compliance with procedures and CARF standards.  Registered Nurse with current license in State of Kentucky.  CPR certification required.  Five years nursing experience with supervisory and / or administrative experience plus demonstrated ability to inspire others to excellence.  At least one year of chemical dependency experience desired.  Additional nursing experience in behavioral health, extended healthcare and acute healthcare preferred. Working knowledge of CARF standards.

Position: Nurse Practitioner

Status: Full Time

The Nurse Practitioner is a specialist who provides primary substance use disorder (SUD) care to patients seeking SUD services in a variety of settings including residential and outpatient treatment programs throughout the Northern Kentucky region. Primary SUD care provided by the nurse practitioner involves the continuous and comprehensive services necessary for the promotion of optimal health, prevention and treatment of substance use  disorders and health maintenance. This includes the assessment, diagnosis, and management of substance use disorders.  Transitions, Inc. is committed to top quality of care to patients, delivered in evidenced based, patient centered and trauma focused interventions. DUTIES AND RESPONSIBILITIES:  Employs evidence-based clinical practice guidelines to guide screening activities, identifies health promotion needs, and provides anticipatory guidance and counseling addressing environmental, lifestyle, and developmental issues.  Under the supervision of a licensed MD/DO and within their scope of practice, the NP assesses, diagnoses, and treats individuals and families with substance use disorders, including the prescription of medication, ordering and monitoring of labs and urine drug screens, and coordinating referrals to other agencies. Monitors for the effectiveness of prescribed medications as well as any adverse effects and manages the treatment of adverse effects. Modifies medications and dosages as indicated during the medication reviews based on the feedback received from treatment team members and symptoms presented by the client. Consults with MD/DO at times of complex medication regimen. Provides interpretation of lab work or other diagnostic tests that have been performed and makes recommendations/diagnoses related to the test results. Makes referrals when needed for additional testing. May help in providing medical consultation directly or by phone to other internal and external agency staff in regards to assessments or findings that may need medical intervention. Provides team members, clients, and families with instructional review/teaching related to medications and health issues. Maintains detailed records on all clinical work and client progress. Has experience using an electronic health record. Participates in on-call rotation to cover for unexpected crisis or emergencies. This may include face-to-face interactions/assessments or a telephone consultation. Collaborates closely with team members, including the nursing and clinical team. Actively participates in team meetings, treatment team consultations, committee meetings, and supervisions. Precepts new employees and students as needed. Has a strong understanding of barriers to access to care and knowledge of community resources.

Position:  Clinical Supervisor – Withdrawal Management Program
Status: Full Time
Provides day-to-day supervision and leadership for direct care staff of the Residential Treatment Center Managed Withdrawal Program.  Works under the direction of the Clinical Director in collaboration with the Lead Registered Nurse.  Performs comprehensive assessments during the admission process to assist in the plan of care, identifying appropriate pathways to additional treatment based on client history.  Coordinates nursing care and education based on client’s individual needs to promote well-being. Acts as the Patient Navigator ensuring patient’s access to care throughout the Residential Treatment Center programs. Master’s Degree in Behavioral Health or related field required. Must be currently licensed by the State of Kentucky as a Marriage Family Therapist (LMFT), Clinical Social Worker (LCSW), Professional Clinical Counselor (LPCC), Marriage Family Therapist Associate (MFTA), Certified Social Worker (CSW) or Professional Clinical Counselor Associate (LPCA). 2 + years of clinical experience working in Behavioral Health, Substance Use Disorder or Co-occurring Disorders programs. 1+ years of administrative supervisory experience required. Experience working with individuals with substance use disorders in a detox and/or residential setting preferred.
Position: Food Service Worker – Cook
Status: Full Time

The food service worker is responsible for ensuring an adequate food preparation. Maintains inventory of all needed supplies including but not limited to menu items snacks, drinks and special needs items for clients (weekday/weekend). Prepares pre-determined weekday lunches and dinners as needed. Monitors cleanliness of kitchen, pantry and cafeteria. Qualifications:   Basic culinary skills, good organization and communication skills, self-motivated.

Position: Certified Medical Assistant – 2nd Shift
Status: Full Time
The Medical Assistant assists providers and other members of the health care team. Performs a variety of patient-related tasks, ensuring smooth patient workflow. Transitions, Inc. is expanding and seeking a Customer Service Specialist for our new Residential Treatment Center! The Customer Service Specialist will be responsible for light office work, answering phones and greeting/directing individuals as they arrive at the Residential Treatment Center.

Position: Intake Coordinator

Status: Full Time

The Intake Coordinator provides professional, client-centered, friendly customer service. The Intake Coordinator will be responsible for registration of clients, payer source verification, benefit application coordinator, insurance authorizations, and coordinating the flow of clients into residential units at the Residential Treatment Center. Qualifications: Associates Degree required, Bachelor’s Degree preferred. Must have a minimum of one year experience in behavioral health, registration, or insurance authorizations.

Position: Employment Specialist

Status: Full Time

Carries out the services of the IPS Supported Employment program by assisting clients to obtain and maintain employment that is consistent with their vocational goals. This is achieved  through collaboration with OVR counselor, treatments teams and ongoing  community engagement with employers. Education and experience equivalent to undergraduate degree in mental health, social services, or business.  Experience working  with co-occurring substance use and mental health diagnosis , experience providing employment services, and knowledge of the work world are preferred. Ability to work independently and as an effective team player is essential

Position: Targeted Case Manager

Status: Full Time

The Targeted Case Manager will provide excellent customer service and maintain a caseload of clients to assist them to more effectively function in the community or to prepare them for reintegration into the community. Bachelor’s Degree in Behavioral Science or related area or, Equivalent combination of experience and training Kentucky Targeted Case Management Training preferred.
